Accessing the Theater Development Fund (TDF) Costume Collection

To access the TDF Costume Collection, Fractured Atlas must submit a form directly to TDF on your behalf. You should visit their website to review their rental policies and procedures. To help us complete the form, please email [e-mail] with the following information:

• Your name or name of the person who will be accessing the Costume Collection
• The name of your sponsored project
• Your affiliation with the project/production (costume designer, director, producer, production/artistic staff, administrative staff, volunteer, or other)
• Telephone number
• Email address
• Best way to contact you (phone or email)
• Have you ever rented from TDF before? (yes or no)
• Show/Performance/Film name
• First preview performance date. (If applicable)
• First official performance (or first shoot date for film)
• Final official performance (or final shoot date for film)
• Type of project (play, musical, opera, dance, film, or other/multi-media)
• Number of seats in performance venue (1-149, 150-299, 300-499, 500+, or Film/TV/Photo Shoot)
• Additional information (is there anything else you think we should know?)
• Will you require any additional services? (fees may apply) Services are:
  
  • Shopping Services (a staff member to personally help you pull costumes)

  • Memo (taking costumes on spec)
  • Mail Order Pulling Services (a staff member to pull costumes based on your design to be mailed to you)(see terms and conditions: https://goo.gl/6XGN4w)

By requesting access to the Costume Collection, you agree to TDF's terms and conditions and you confirm that you are authorized by your project/company to rent on their behalf. Please be sure to review TDF's terms and conditions before you request access to rent. 

Upon visiting the Costume Collection, you will be asked to complete a Credit Card Authorization form for any charges related to rental, so please be sure you have that information on hand during your visit.
