W9 Form

Requirements for Obtaining a W9 

Donors and other entities may ask for a W9 in order to remit payment of a donation to Fractured Atlas for the purposes of your sponsored project. Any time a donor, venue, institution or other entity is requesting documents, you must obtain these documents from Fractured Atlas.  

An email must be sent to [e-mail] with full information of the institution requesting the form and details of the circumstance in which is it needed.   

Fractured Atlas W9 documents and information cannot be used to:  

• Open bank accounts. Fractured Atlas cannot provide any documents to open a bank account, and the project must maintain all bank accounts separate from Fractured Atlas. 
• Receive earned income from services rendered or sold goods. All earned income must go to the project directly.
